Required Forms
(Please fill out and send to camp prior to the 1st day of the session.)
- Health History and Physical Form - Please fill out the
first 2.5 pages, obtain an immunization record from your doctor's office,
and a record of your daughter's last physical exam (conducted within 24
months of camp attendance). The physical exam record needs to be
signed by the doctor. The exam record and the immunization record
can either be included on this form or the doctor's office can supply this
information on their own form.
- Medication Permission Form - Both Parent AND
Physician must sign. Any prescription and over-the-counter
medications that you would like administered to your child IN CASE the
nurse is not available must be included on the back side of the form.
